A freshwater fish would presumably have less salt in its blood than would be present in the surrounding water. Diffusion only moves molecules from an area of high concentration to one of lower concentration. Movement of salt from the lower concentration, water, into the area of higher concentration, blood, requires active transport and the use of energy.
