How does Claire realize that Geillis is from the future?
Prior to their escape from prison, Claire realizes that Geillis is from the future because of the Smallpox vaccine scar on her arm. The Smallpox vaccine was created in the late 19th century by a pharmaceutical company, so it would be impossible for anyone not from the 20th century or later to get the Smallpox vaccine.

Why do Frank and Claire go on a second honeymoon?
After working apart for much of World War II (Frank was an intelligence officer in MI6), Frank and Claire decide to go on a second honeymoon to the Highlands of Scotland. Frank was interested in conducting genealogical research; Claire was interested in gathering plants near standing stones.
Ultimately, the two decided to go a second honeymoon to grow closer to each other and rekindle their relationship after a long time apart from each other.

Why does Colum MacKenzie suspect Claire to be a spy?
Because Claire has advanced medical skills and is able to help quite a few people, she gains the respect of a number of villagers. However, not everyone trusts her. Colum MacKenzie, in fact, doesn't trust her at all and thinks that she is an English spy. MacKenzie thinks that Claire is a spy because she is ignorant of Gaelic culture (and is called an "outlander") and because Claire acts in a strange way. MacKenzie questions Claire a number of times, but ultimately stops suspecting her after she marries James.
Not only did James and Claire get married. In fact, Claire got pregnant with James' baby.
