Medicare for All
One of the key themes in Sanders' book and his campaign for President of the United States is Medicare for All.
In Sanders' view, healthcare is a human right. He believes that if the United States were to guarantee free healthcare (in the form of Government-subsidized Medicare for everyone), the world would be a much better place, lifespans would increase, and the quality of life for Americans would be dramatically and substantially increased.
Sanders contends since most of the rest of the world has free healthcare for all of its citizens, the United States should have Medicare for All. And despite its tremendous cost (Sanders himself admits it would cost between 30 and 40 million dollars over the course of ten years to finance), Sanders thinks its benefits heavily outweigh its negatives.
Climate Change
For much of his life, Sanders has said that Climate Change is an "existential threat" to humanity. He believes that if humanity doesn't dramatically reverse course and stop climate change from progressing, the world would be in a much better place.
In the book - and throughout his campaign - Sanders chastised President Donald Trump for promising to and later leaving the Paris Climate Accords, which Sanders believes is a needed start to help turn the tide of Climate Change.
In no uncertain terms, Sanders says that an effort akin to the Marshall Plan must start in order to save the planet and by proxy, the human race from tremendous pain, suffering, and most importantly, horrific and unnecessary death.
An Older Man with "Radical" Ideas vs. a Country Wary of Socialism
Beyond everything else, though, the prevailing theme of Sanders' book is the conflict between an older man with seemingly "radical" ideas and a world which is still wary of Sanders' Socialist ideas.
For example, Sanders' plan for free college tuition is, by virtue of public approval ratings, anathema to what many Americans want. Many think Sanders' plan is unrealistic and irresponsible, but he wants to change the worlds mind. To that end, he fights tooth and nail against a country wary of Socialism to help people understand his point of view.
