The book follows the real-life story of the transgender political activist and actress, Nicole Maines.
Nicole was born biologically as a male, known as Wyatt, and has a twin brother, Jonas Maines. Growing up, they were given up for adoption o their maternal aunt, Kelly, and her husband Wayne. The boys have a happy childhood although obvious differences in personality are seen by the two. While Jonas prefers more traditionally masculine activities, Wyatt prefers to engage in more feminine play.
It is clear that Wyatt wishes to be a girl and begins to dress like one. This brings him into conflict with Wayne, who cannot handle the idea of his son transitioning to a girl. Moreover, Wyatt begins to experience some bullying at school over the way he dresses, which worries Kelly and Wayne. Needing some medical guidance, Kelly contacts pediatric endocrinologist Dr. Norman Spack, who helps Wyatt medically transition suing drugs.
Nonetheless, Wyatt, now known as Nicole, experiences difficulties at school. This came to prominence and gained international recognition when there was difficulties in her accessing the girls bathroom, seen in the infamous Doe v. Clenchy (2013) trial.
The book follows onto Nicole’s difficulties in establishing romantic relationships, and the break in the relationship with her twin brother Jonas. There is also an insight into Nicole’s emotions when it came to her sex reassignment surgery.
