Darkness
The set of this teleplay is incredibly dark, with dark lighting and furniture. Overall, this reflects the dark, disturbing and uncomfortable mood of the teleplay and the strange relationship between Elizabeth and Glen. In addition to this, this imagery also represents the fact that the audience is metaphorically in the dark about whether or not Glen is a figment of Elizabeth's imagination.
The Fire
Glen describes a fire that happened at his adoptive parents' house. He tells Elizabeth:
"They're both dead. The house burned down. Swish, crackle... Bye-bye Brennans."
This is a disturbing way to describe a house fire, which suggests something about Glen's disturbed state of mind. The fact he uses imagery like "swish" and "crackles" suggests he is pleased about the fire.
Hospital imagery
Glen also uses imagery to describe his tedious stay at the hospital. As he refused to speak to anyone, a worker at the hospital was asked to speak to talk to him for a while. Glen uses a lot of imagery to describe this experience, comparing the man to a goldfish and a woodpecker. Overall, we get a strong impression that Glen was annoyed about the conversation.
