Atticus suspects that it was Bob Ewell that caused Mayella's injuries. Atticus subtly suggests that Bob Ewell has been raping his own daughter. Bob's wife died many years ago yet Bob still has many small kids.

Atticus suspects that it was Bob Ewell that caused Mayella's injuries because Tom Robinson's left arm is messed up and he can't do anything to Mayella with that arm. Also Mayella's right side was brused meaning that someone who is left handed would have to have beat her up.
