Dr Begbie (Cate) is assisting Ruby because she is a part of the Children's League. She isn't really a doctor and has been sent to find children who are "oranges" and bring them to the league. The Children's league poses a great danger to Ruby, as most of the "oranges" have been imprisoned or killed.
