Where is Adam when the transcribed tapes are recorded and how do you know?

Adam is in the hospital. We don't learn this for sure until the final chapter when Paul is greeted by a member of the hospital staff and escorted to his room. It is only then that we realize he's been hospitalized from the beginning.
