Blanche is pretty desperate at this point. Shep was a former suitor of Blanche's whom she met again a year before her arrival in New Orleans while vacationing in Miami. Shep is actually married now but Blanche hopes Shep will help her and Stella escape Stanley. I think Shep has moved past whatever interest he might have had in Blanche.
